What is Chronic Disease?

Chronic diseases are defined broadly as conditions that last 1 year or more and require ongoing medical attention or limit activities of daily living or both. Chronic diseases such as heart disease, cancer, and diabetes are the leading causes of death and disability in the United States. 

Many chronic diseases are caused by a short list of risk behaviors that include:

Tobacco use and exposure to secondhand smoke.

Poor nutrition, including diets low in fruits and vegetables and high in sodium and saturated fats.

Lack of physical activity.

Excessive alcohol use.


What can Family Care Do for My Chronic Disease 


If you have any chronic disease, we are here to help you with individualized treatment plans.  Our job is to make sure you are educated on your disease process and focus on treatments that are not only effective but go along with your belief system.  Our providers are not just Board Certified but are culturally sensitive to their approach on our patients' illnesses.

Do I have to take Medications For The Rest of My Life?


We believe that some chronic diseases can be managed without medications. However, depending on your diagnosis, you may need to take medications for a long time.  The first step is to get an appointment with our Doctors and see what treatment options you have available.
